Cygnet PCN High Wycombe is a caring and innovative Primary Care Network (PCN). We are focused on delivering exceptional care to all our patients within Buckinghamshire.

What is a PCN?
A Primary Care Network (PCN) is a group of GP practices working together with each other and in collaboration with other healthcare providers such as hospitals, pharmacies, community and social care, mental health and voluntary services.
These services all work together with the aim to provide more coordinated care, enhanced access to services and to address the specific health needs of a population who are now living longer. Significantly Cygnet PCN is based within the local area of Buckinghamshire.
How does this benefit the patients?
A GP Practice which is part of a PCN means that there are larger teams of staff to help you. For example, it may be more appropriate for you to see a different clinician such as a Paramedic or a Clinical Pharmacist, who can treat your health needs and it’s likely they will be able to see you sooner. As your local health services have access to your health records, you won’t have to explain your story or repeat health history time and time again. Our collective PCN team can provide a wide range of different staff to meet the varying needs of our patients.


Integrated Neighbourhood Teams (INTs)
Health and care services are changing — and for the better. Following the Fuller Stocktake report (May 2022), NHS England recognised the need to bring health and care services closer to home, make them more joined-up, and improve people’s access, experience, and outcomes. The goal is to create a more sustainable system that works better for everyone.
