Our Wellbeing Team
Looking after your health isn’t just about medication or appointments — it’s also about feeling well in yourself, staying connected, and having the right support when life gets a bit tough.
That’s where our Wellbeing Team comes in.
Each of our four member practices has its own Social Prescriber, and they also work together across the PCN to make sure patients get the support they need. Social Prescribers are here to help you with the things that might be affecting your health and wellbeing, like:
- Feeling isolated or lonely
- Struggling with housing, finances, or work
- Managing stress, anxiety or low mood
- Finding activities or support in your local area
- Navigating caring responsibilities or long-term conditions
They’ll take the time to chat with you, understand what matters to you, and connect you to the right support — whether that’s a local group, service, or something more tailored to your needs

The Freshwell Low Carb Project
A fantastic resource for people interested in healthy eating and managing weight, prediabetes, type 2 diabetes, and other metabolic health concerns. Developed by a GP team, it includes easy-to-follow advice, recipes, and downloadable guides.
