Health and care services are changing — and for the better. Following the Fuller Stocktake report (May 2022), NHS England recognised the need to bring health and care services closer to home, make them more joined-up, and improve people’s access, experience, and outcomes.

The goal is to create a more sustainable system that works better for everyone.

Integrated Neighbourhood Teams, or INTs, are groups of professionals from across the health and care system who work together to support the needs of local communities.

This includes people from:

  • GP practices,
  • Hospitals,
  • Community health teams,
  • Mental health services,
  • Social care,
  • Charities and local voluntary organisations.

Rather than working in separate silos, these teams collaborate to provide more coordinated, personalised care — often in or near people’s homes. This new approach helps people get the support they need, when and where they need it, and aims to improve health and wellbeing across the whole community.

At Cygnet PCN, we are working closely with our partners across Buckinghamshire to shape what Integrated Neighbourhood Teams will look like in our area. This is an exciting opportunity to redesign care in a way that better meets the needs of our patients and local population.

Therefore, Cygnet PCN will be working with another local PCN, Dashwood, in the proposed High Wycombe INT.
