The Clinical Directors provide strategic clinical leadership across the Primary Care Network, working together to shape local services and improve patient care. As joint Clinical Directors, they work closely with GP leads, the Network Manager, and Practice Managers to develop and deliver PCN priorities, support innovation, and ensure services meet the needs of the local population. They also represent the PCN at system-level meetings, help lead on quality improvement and play a key role in supporting multidisciplinary working across member practices.

The Network Manager plays a central role in keeping the Primary Care Network running smoothly day to day. Working closely with the Clinical Directors, GP leads, and Practice Managers, they oversee recruitment and line management of PCN staff, coordinate service delivery, and manage the network’s finances, including budgets and funding streams.

They also lead on writing and coordinating funding bids to support new initiatives and services. The role helps bring teams together, supports strategic planning, and ensures the PCN can deliver high-quality, joined-up care for patients.

A Primary Care Network (PCN) pharmacist works alongside a multidisciplinary diverse team of health care professionals in general practice and within the PCN.

Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.

Pharmacy technicians work within a multidisciplinary team of healthcare professionals, based within a GP practice supporting the delivery of safe, effective and efficient systems for medicines optimisation, repeat prescribing, reducing medicine waste and maximising patient outcomes.

Pharmacy technicians work under the supervision of a pharmacist and are registered healthcare professionals that are responsible and accountable for their own accurate and safe practice.

Paramedics are an integral part of the primary care multi-professional team within general practice. Paramedics will provide care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. Our paramedics also typically conduct any home visits required.

Emily Nash (Priory Surgery)

A Mental Health Practitioner is a specialist mental health practitioner who is an experienced professional who can support patients with either a diagnosed or undiagnosed mental health concern. Mental health practitioners are based in GP surgeries or neighbourhood settings to:

  • support people with severe mental illness or complex mental health needs
  • act as a ‘bridge’ between primary care and specialist mental health services
  • provide GPs and other primary care staff with timely support and advice
  • build stronger relationships with mental health services

Social prescribers connect people to non-medical support to address these issues and other unmet needs. Many things that affect our patients health can’t be treated by doctors or medicine alone. Issues such as loneliness, debt, or stress due to financial pressures or poor housing.  Social prescribers can help change the circumstances that can make people unwell. It can empower people to manage existing health problems, to get the right benefits or get back into employment. It can help people to connect and to grow in confidence.

Care co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. They can support people to become more active in their own health and care and are skilled in assessing people’s changing needs.

Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs. They can be an effective intervention in supporting people to stay well particularly those with long term conditions, multiple long-term conditions, and people living with or at risk of frailty.

Our PCN Board includes a GP Partner and Practice Manager from each of our four practices, along with our Network Manager. We meet face to face every month to plan how we work together as a PCN. The Board helps us stay joined up, make decisions as a group, and run services that make sense for our patients and community.
